For and in consideration of Beverly Hills Tennis sponsoring the above tennis classes, I agree to release and forever discharge Beverly Hills Tennis, the City of Beverly Hills, its officers, agents, and employees from any and all liabilities, demands or claims for loss or damage resulting from any injury or damage.

I acknowledge there are no refunds, or credits for injury, illness, or missed classes. There are no makeup classes.

Appropriate levels will be determined by the instructor, and potential class changes may be made.

Schedules and instructors are subject to change.

I hereby authorize Beverly Hills Tennis to publish pictures from camp/classes for printed publications and online usage. I acknowledge that since my participation in both printed and online publications produced by Beverly Hills Tennis is voluntary, I will receive no financial compensation.
